Loan Forgiveness

There are many applicable programs for student loan forgiveness available to public educators.  The Human Resources Department will help complete the employer portions of the forms, whether it is digital or a paper copy.

Process for Student Loan Forgiveness Forms

1.  Complete the applicant portion of the form, then send a copy to Janelle Gray, Administrative Assistant in the Human Resources Department.  These can be sent via inneroffice mail or emailed (jgray@fortosage.net) 

*Please do not fill out any part of the application that is to be filled out by the district. 

2.  Our team will either submit the form if electronic, or send back to you to mail the form to the appropriate location.
