Consiglio Wellness Center

The Consiglio Wellness Center serves the staff of the Fort Osage School District.

Before your first visit, you will need employee identification and your Cigna health insurance card. Minors and dependents under the age of 18 must be accompanied by a parent for their first visit to the office. They may be unaccompanied at future visits with parent/guardian consent for treatment. To expedite the new patient registration process, call the office at (816) 224-1740.

Friday, 7:00 a.m. - Noon
Saturday, 8:00 a.m. - Noon

 *The office closes for lunch from Noon - 1:00 p.m.

Sports Physicals

The CWC is pleased to offer Student Sports/Activities Physicals in compliance with MSHAA. If your son or daughter requires a sports physical, simply contact the CWC at 816-224-1740 to make an appointment. Drop-ins are always welcome but an appointment insures shorter wait times if the wellness center happens to be busy.

A physical form is available to download and print at home or you can obtain a form at the center with your appointment.
